<p class="wp-block-paragraph">C&rsquo;est un mécanisme que la majorité des experts médicaux connaissent, mais dont vous ignorez surement l&rsquo;existence : le stress oxydatif. Ce déséquilibre, souvent négligé, pourrait bien être la cause pour comprendre ce que vos follicules subissent. Et pourquoi, finissent-ils par vous abandonner ?</p>

<p class="wp-block-paragraph">Le stress oxydatif se produit lorsque votre corps génère plus de radicaux libres  des molécules instables qui endommagent les cellules  qu’il ne peut en neutraliser avec des <a href="https://believehaircare.com/soin-des-cheveux/what-does-antioxidant-mean/">antioxydants</a>. Ces radicaux libres peuvent attaquer les protéines, l’ADN et les lipides, y compris ceux situés à l’intérieur de vos follicules pileux. Lorsque cela se produit au niveau du cuir chevelu, cela entraîne une inflammation, un dysfonctionnement cellulaire, et avec le temps, l’affaiblissement voire la mort des cellules responsables de la production de cheveux.</p>



<p class="wp-block-paragraph">Et voici où cela devient personnel. Vous n’avez pas besoin de mener une vie extrême pour être concerné. Si vous vivez dans une ville fortement polluée, passez des heures en plein soleil sans protection pour le cuir chevelu, consommez une alimentation riche en sucres ou en aliments ultra-transformés, ou subissez un stress chronique félicitations, vous êtes déjà submergée par le stress oxydatif sans même vous en rendre compte.</p>

<p class="wp-block-paragraph"> À la base de chaque follicule, on trouve la papille dermique, un petit noyau de cellules, fragile et  essentiel.  C&rsquo;est à cet endroit que la pousse prend racine. Ces cellules travaillent non-stop, elles consomment de l’énergie, elles régulent la production de kératine, elles répondent à des signaux hormonaux, nutritionnels, nerveux. Tout se joue là. </p>

<p class="wp-block-paragraph">Dans le cas de <strong>l’alopécie androgénétique</strong> les chercheurs ont observé une chose troublante : peroxydation lipidique  très élevée dans le cuir chevelu. </p>



<p class="wp-block-paragraph">Dit autrement ? Les lipides &#8211; graisses naturelles qui protègent vos cellules &#8211; s&rsquo;oxydent, un peu comme un fruit qui brunit à l&rsquo;air libre. C&rsquo;est ce qu&rsquo;on appelle des dommages oxydatifs, et ici, ils sont bien visibles au cœur même des tissus de votre cuir chevelu.</p>



<p class="wp-block-paragraph"> Et ça change tout. Parce que ce qu&rsquo;en plus d’aggraver l’effet de la DHT &#8211; l’hormone qui affine votre cheveux &#8211; ce stress oxydatif semble aussi faire vieillir les follicules plus vite.</p>

<p class="wp-block-paragraph">Face à ce déséquilibre votre cuir chevelu réagit :  il raccourcit brutalement le cycle de vie de votre cheveux et le pousse à basculer de façon précoce dans la phase de chute. Résulat : des poignées de cheveux qui tombent, parfois sans signe avant-coureur.</p>

<p class="wp-block-paragraph">Prenez <strong>la vie urbaine</strong>, par exemple. Être coincée dans les embouteillages, respirer un air saturé en particules fines, marcher tous les jours sous un ciel gris chargé en dioxyde d’azote… c’est une agression permanente pour votre cuir chevelu. Des études menées sur des populations urbaines en Chine ou en Inde ont clairement établi le lien entre pollution atmosphérique, amincissement capillaire et cheveux grisonnants précoces. Et ce n’est pas réservé à l’Asie. Paris, Marseille, Dakar, New York, c’est pareil : le cocktail polluant n’épargne personne.</p>

<h4 class="wp-block-heading">Vos cheveux brillent, mais vos follicules crient</h4>



<p class="wp-block-paragraph">Ajoutez à cela les <strong>appareils chauffants et traitements chimiques</strong> : plaques lissantes, brushing à répétition, colorations à base d’ammoniaque ou décolorations en salon… tout ça génère des radicaux libres. Pas uniquement sur le cheveu visible, mais jusque dans l’environnement folliculaire. Résultat : vous pouvez avoir des cheveux brillants sur Instagram, mais une racine déjà oxydée en profondeur. Et cette racine, elle ne ment pas.</p>

<p class="wp-block-paragraph">Autre agresseur majeur : le <strong>soleil</strong>. Les rayons UV ne s’arrêtent pas à la peau. Ils attaquent aussi la structure interne du cuir chevelu, dégradent la kératine, oxydent la mélanine, et déclenchent des processus inflammatoires. Beaucoup de personnes qui passent leurs étés à la plage, tête nue, finissent par remarquer des cheveux plus ternes, cassants, et parfois une perte plus diffuse en fin de saison. C’est rarement une coïncidence.</p>

<p class="wp-block-paragraph">Et puis il y a <strong>le stress émotionnel</strong>, le vrai. Celui qui dure. Celui qui vous fait ruminer la nuit, qui vous coupe l’appétit ou vous pousse à tout avaler sans réfléchir. Ce stress-là fait grimper le cortisol, et le cortisol, lui, baisse vos défenses antioxydantes. Le cuir chevelu, déjà vulnérable, devient alors une cible facile. Vous ne le sentez pas tout de suite. Mais les semaines passent… et la chute commence.</p>

<h2 class="wp-block-heading"><strong>What is alopecia areata?</strong></h2>

<p class="wp-block-paragraph">Alopecia areata is<strong><a href="https://academic.oup.com/bjd/article-abstract/179/5/1033/6732534?login=false"> a non-scarring form of hair loss</a></strong>—“Non-scarring” matters here because it means that the hair follicle itself is usually preserved. Your hair has the potential to grow back, even after significant shedding.</p>



<p class="wp-block-paragraph">It usually appears as small, round or oval bald patches across the scalp and can recur over time, sometimes without visible inflammation or other symptoms. One day your scalp looks perfectly ordinary, the next, your hairline decides to experiment with negative space. <em>Charming, right?</em></p>

<p class="wp-block-paragraph">A lot of the time, the explanation of alopecia areata stops at: “It’s an autoimmune disease that attacks your hair follicles.” Clean, simple, easy to summarize in a sentence or two. </p>



<p class="wp-block-paragraph">The problem is that the actual mechanisms behind alopecia areata are far more complex, and outside of dense scientific papers where every third word seems determined to require another browser tab, they’re rarely explained in a way that feels both understandable and genuinely readable.</p>

<h5 class="wp-block-heading" style="text-decoration:underline">The autoimmune nature of alopecia areata</h5>

<p class="wp-block-paragraph">Alopecia areata is widely considered an autoimmune condition in which your immune system mistakenly targets your hair follicles. Which, admittedly, sounds suspiciously similar to every other alopecia areata explanation you’ve probably already read online. But stay with us, there’s more. </p>



<p class="wp-block-paragraph">Several immune cells appear to be involved in this process. Researchers believe this immune reaction may disrupt the normal hair cycle and thus contribute to hair loss.</p>



<p class="wp-block-paragraph">One of the main questions surrounding alopecia areata is understanding why the hair follicle becomes interpreted as dangerous in the first place. Some hypotheses suggest that changes within the follicle environment may trigger abnormal immune reactions, while others point toward a broader dysregulation of the immune system itself.</p>

<h5 class="wp-block-heading" style="text-decoration:underline">Why does your hair follicle suddenly become the enemy?</h5>

<p class="wp-block-paragraph">Your immune system does not attack your body blindly. Before a reaction happens, immune cells need signals telling them that something may be wrong. This is the role of receptors, like NKGD2+ which act as detection tools helping immune cells identify stressed or abnormal cells.</p>



<p class="wp-block-paragraph">Once those damaged cells are detected, information about them begins circulating through your immune system so it can determine whether a response should be triggered. Part of this communication process has a scientific name which is, antigen presentation.</p>



<p class="wp-block-paragraph">Think of antigen presentation concept, as cells holding up an evidence card to the immune system, signaling that something may be dangerous. In alopecia areata, one hypothesis is that this warning system may become misdirected around your hair follicle.</p>



<p class="wp-block-paragraph">During the hair cycle, your follicle cells go through a natural process. They die, shrink, and restructure, especially during the catagen phase, this process is called <strong><a href="https://onlinelibrary.wiley.com/doi/full/10.1111/exd.12209">apoptosis</a></strong>. In alopecia areata, the problem may begin if fragments from those dying follicle cells are presented in a way that makes your immune system interpret the follicle as suspicious.</p>



<p class="wp-block-paragraph">From there, several immune cells, like CD8+ T cells or NK cells among others normally responsible for detecting and responding to abnormal cells, may begin reacting around the follicle.</p>



<p class="wp-block-paragraph">In other words, your hair follicle may not be “dangerous” in itself. It may be caught in a faulty immune conversation — and as usual, the body rarely asks for clarification before escalating.</p>

<h5 class="wp-block-heading" style="text-decoration:underline">Why does alopecia areata affect people differently?</h5>

<p class="wp-block-paragraph">Although the exact causes of alopecia areata remain unclear, researchers have identified several factors that may contribute to its onset. Genetic predisposition appears to play an important role, particularly since alopecia areata frequently coexists with other autoimmune conditions. In other words, the immune system rarely develops questionable decision-making habits in complete isolation.</p>



<p class="wp-block-paragraph">Environmental and hormonal factors are also believed to influence the condition, alongside stress and broader immune dysregulation. Some people may experience only a few isolated patches during their lifetime, while others develop recurrent or more extensive forms of hair loss. Which is precisely why researchers still disagree on whether alopecia areata follows one single disease mechanism or several overlapping ones depending on the patient.</p>

<p class="wp-block-paragraph">Alopecia areata is often presented as an immune system attacking a hair follicle. While technically true, that explanation only captures part of the picture. What researchers have uncovered instead is a condition shaped by immune signaling, biological predispositions and mechanisms that are still being debated today. Which may also explain why alopecia areata rarely follows the same path twice, even among people living with the very same diagnosis.</p>

<p class="wp-block-paragraph">The term « ophiasis » derives from the ancient Greek word ophis, meaning snake. In medicine, the term « ophiasis » is used to describe a pattern of hair loss that resembles a snake around the scalp.</p>



<p class="wp-block-paragraph">In this type of alopecia areata, hair loss appears in bands or lines along the edges of the scalp, often taking the shape of a horseshoe or a snake.</p>

Most treatments aim to calm the immune reaction surrounding the hair follicle and encourage the hair cycle to resume more normally. Results, however, vary enormously from one person to another. Some experience significant regrowth, others move through recurring cycles of shedding and regrowth for years. Your immune system, unfortunately, does not always believe in consistency.</p>

<h5 class="wp-block-heading" style="text-decoration:underline">The first line of defense against alopecia areata</h5>

<p class="wp-block-paragraph">The most commonly prescribed treatments for alopecia areata are <strong><a href="https://link.springer.com/article/10.1007/s13555-025-01421-2">corticosteroids.</a></strong> Depending on the severity and extent of hair loss, corticosteroids can be administered in different ways.</p>



<p class="wp-block-paragraph">Intralesional corticosteroids, which are corticosteroid injections administered directly into bald patches, are often considered among the most effective treatment options for alopecia areata.</p>



<p class="wp-block-paragraph">On the other hand, topical corticosteroids are applied directly onto the scalp, while oral systemic corticosteroids circulate throughout your entire body through the bloodstream and are generally reserved for more extensive cases.</p>



<p class="wp-block-paragraph">Corticosteroids are medications that mimic cortisol, a hormone naturally produced by your adrenal glands. Their role is to reduce inflammation and calm excessive immune activity, which is why they are widely used in inflammatory and autoimmune conditions ranging from asthma to psoriasis.</p>



<p class="wp-block-paragraph">In alopecia areata, they are mainly prescribed to reduce the immune response believed to target your hair follicle.</p>

<h5 class="wp-block-heading" style="text-decoration:underline">When your immune system requires heavier intervention</h5>

<p class="wp-block-paragraph">In more severe forms of alopecia areata, stronger immunosuppressive treatments may also be considered. <strong><a href="https://link.springer.com/article/10.1007/s13555-020-00370-2">Cyclosporine</a></strong>, for example, was originally developed to help prevent organ transplant rejection before being used in several autoimmune and inflammatory diseases, including severe alopecia areata.</p>



<p class="wp-block-paragraph"><strong><a href="https://www.sciencedirect.com/science/article/abs/pii/S0190962218322230">Methotrexate,</a></strong> another medication sometimes prescribed in alopecia areata management, works by slowing cell growth and suppressing immune activity.</p>



<p class="wp-block-paragraph">These treatments, however, are not without risks. Cyclosporine in particular may cause <strong><a href="https://www.frontiersin.org/journals/pharmacology/articles/10.3389/fphar.2024.1453034/full">significant side effects, </a></strong>including kidney toxicity, high blood pressure, liver toxicity and neurological complications affecting the nervous system</p>



<p class="wp-block-paragraph">Which is also why treating alopecia areata is rarely as simple as applying a miracle serum and waiting for spiritual enlightenment by week three.</p>
